To receive our services, please have your physician complete the Patient Diagnosis Verification Form below and submit it to us for approval. Once completed, you will be assigned an account number. When arranging services with our vendors, please provide your account number with Hope Floats. We are always looking for volunteers to help with major fundraising events, hosting a fundraiser with your family or friends, or even back end help for fielding calls and talking with patients who may want more information regarding our organization.
